It’s true, dermal fillers can make you look older, but only if they aren’t administered properly. Overfilling, treating areas in isolation and avoiding dissolving when necessary can make you look older. 

Fillers don’t make people look older. Poorly placed, poorly planned filler does.

When used incorrectly dermal filler results can cause you to appear older. When used thoughtfully, dermal fillers can restore lost volume, soften shadowing and gently rebalance facial proportions. Choosing the right cosmetic injector, whose values align with your own, is crucial for getting the natural-looking dermal filler results you want. 

I pride myself on providing my patients undetectable dermal filler. Dermal filler is not about simply adding volume—it can smooth static lines, restore support and improve symmetry. My approach to dermal filler, and all cosmetic injections, is rooted in anatomy, balance, and restraint.

That said, I completely understand where the fear around dermal filler stems from. We’ve all seen photos of overfilled faces online that feel exaggerated or heavy, and those images stick. Over time, poorly placed filler has become linked with looking older rather than refreshed. I don’t dismiss those concerns—they’re valid, and poorly executed filler does exist. But it’s important to know that those outcomes aren’t caused by filler itself. They’re the result of how much is used, where it’s placed, and the intention behind the treatment.

How can dermal filler make you look older?

Most of the fear around filler aging the face comes from seeing results that don’t look natural. When too much volume is placed everywhere there’s a line or a bit of sagging, it can blur the face’s natural contours. Cheeks can start to look heavy, angles can appear sharper, and new shadows can form where they didn’t exist before. In the lips, overfilling can sometimes lead to migration—creating that recognizable fullness above the lip border that draws attention to the treatment instead of the person.

Why does filler placement matter so much?

Hyaluronic acid–based fillers, like the Revanesse® family, are designed to integrate seamlessly into the skin. The substance itself—hyaluronic acid (HA)—is biocompatible and mimics what’s already in your skin. They move with your expressions, attract and hold water, and improve hydration and volume in a way that looks soft and natural when the right product is used in the right place. 

Those natural looking results require thoughtful placement, though. Restoring volume in one area often means considering its neighbouring structures. For example, adding volume to the lips without supporting the chin can make the chin appear recessed. Treating mid-face volume loss without addressing the temples or jawline can throw off balance and create unnatural contours. Facial harmony comes from understanding how each area works together.

Facial balancing also means understanding when not to treat certain areas. If you already have full cheeks, adding more volume could make your face look wider; instead, supporting the temples or chin might achieve a better result. By looking at the big picture, we create improvements that are undetectable yet transformative.

Can too much filler make you look worse?

When filler is overused or placed without a clear plan, it can absolutely work against you. The issue isn’t dermal filler itself. It’s using too much product, in the wrong areas, or trying to solve every sign of aging with volume alone. When filler is repeatedly added to the cheeks to “lift” the face, it can actually weigh the tissues down, creating that rounded, heavy look often referred to as “pillow face.” Natural curves flatten, shadows shift, and the face can start to look less defined rather than more youthful.

Overfilling areas like the lips, jawline, or under-eyes can blur the clean lines that give the face structure. Instead of restoring support, excess volume can make features appear swollen or undefined. In areas like the nasolabial folds, marionette lines, or chin, too much filler can create sharp or bulky planes that catch light in unnatural ways and feel harsh on the face.

As both a cosmetic injector and a Naturopathic Doctor, my approach has always been conservative. As I’ve shared previously: I don’t believe anyone needs filler. If you’re unsure about the treatment, it’s better to hold off.  And when we do decide to proceed, less is almost always more. Building volume slowly allows us to refine your results, avoid migration and maintain natural proportions.

Dissolving Filler | What to do if you dislike your filler results

If you’ve had filler done elsewhere and don’t love the way it looks, you’re not stuck with it. Even when filler is placed well and conservatively, hyaluronic acid fillers can migrate or shift over time as the face moves, ages, and changes, and in many cases less truly is more for maintaining natural balance. HA fillers aren’t permanent, and an enzyme called hyaluronidase can be used to gently dissolve filler when there’s excess volume, migration, or results that simply don’t feel like you. The goal is never to erase everything or over-dissolve, but to remove only what no longer serves your features. Sometimes dissolving a small amount allows us to reset and rebuild more thoughtfully, with better placement and proportion, restoring harmony rather than adding more. What other options are there for improving volume loss?

For patients who’ve experienced more significant volume loss—whether from natural aging or after dissolving overdone filler—there are options beyond simply adding more filler. One of my favourites is Sculptra®.

Unlike traditional dermal fillers that add volume by occupying space, Sculptra works by stimulating your own collagen production. It supports the skin from within, gradually rebuilding structure over time. Results appear slowly, over several months, which patients often appreciate—it allows for a very natural transition rather than an obvious change all at once. Because we’re encouraging your body to do the work, Sculptra® also avoids the heaviness that can come with too much hyaluronic acid filler.

Once balance has been restored and collagen production is underway, we can thoughtfully layer in small amounts of HA filler where refinement is needed.

What makes one injector’s results look more natural than another’s?

When it comes to cosmetic injections, who does your treatment matters. Technique, training, and philosophy all play a major role in whether results look natural—or end up feeling off.

Your results depend heavily on who performs the injections. Look for someone with medical training and credentials—in British Columbia, only physicians, dentists or registered nurses with specific training can inject products like Sculptra®. It’s also worth asking about continued education, because cosmetic medicine evolves quickly; I regularly attend courses on the latest products and techniques to keep my patients safe and offer the most effective options.

I also believe perspective matters. As a Naturopathic Doctor, I approach cosmetic injections with a holistic lens. How we feel about our appearance can affect confidence, mood, and quality of life—and those things don’t exist in isolation. That’s why my practice blends cosmetic and wellness care. It’s not about chasing trends or changing faces; it’s about thoughtful support that aligns with how you want to feel.

Your injector should value balance and restraint, and be comfortable saying no when something doesn’t make sense for your face. Treatments should always be guided by your comfort level, your goals, and what’s actually appropriate—not what’s popular on social media.
